HEALTHACUITY PANEL MEMBER PRIVACY POLICY

Your privacy is very important to us. HealthAcuity is a trademark of Segmedica, Inc.  This policy is administered by Segmedica, Inc..  This Privacy Policy for panel members is intended to meet or exceed the privacy standards of the Council of American Survey Research Organizations ("CASRO"), and the requirements of federal and state laws and regulations. (CASRO's Code Of Standards And Ethics For Survey Research is available at www.casro.org).

As a panel member you have agreed to provide certain information to Segmedica. Some of this information may reveal your identity. Segmedica will not release any identifying information without your written permission, unless Segmedica is legally required to release this information by court order or some other legal requirement. A minimal amount of identifying information may be disclosed to the company for which this survey is being conducted when necessary to validate interviews and/or to determine an additional fact of analytical importance to the study.

Segmedica may share information provided by you with representatives of the company for which this survey is being conducted and other researchers. Segmedica will use information provided by you only for legitimate survey research purposes. Information will not be disclosed to anyone for the purpose of generating business leads.

The HealthAcuity website does not use "cookies". Cookies is a feature used by some websites which assigns a unique number to the computer a visitor uses to access a website for future identification.

If Segmedica becomes aware of an adverse event experienced by a panel member, Segmedica may have a legal obligation to report this (including identifying information about the panel member) to the company for which the survey was conducted or to a government agency. Adverse events include death, a life threatening condition, a condition which causes a disability, or a condition which results in hospitalization.

Any changes in Segmedica's Privacy Policy will be communicated to panel members as soon as possible.
